Is able to respond quickly and accurately to changes in condition or response to treatmentCore Job Responsibilities• Support the PCP in the delivery of care by gathering and documenting relevant clinical data, as appropriate and per patient need.• Participate in the daily huddle to identify individual patient needs for upcoming appointments.• Clinical liaison between patient and provider for questions, concerns and testing results.• Communicate effectively with other members of the care team in coordinating the individualized plan of care.• Support PCP during in office procedures and examinations.• Provide outreach and education to patients in need of services as identified through quality improvement initiatives (Population Health Management).• Perform duties within scope of practice to the highest level of licensure.• Provide direct support & guidance to other members of the care team as appropriate.• Able to perform assessment data collection on all patients utilizing tools / forms in EMR.• Adequately assess and reassess pain.
